Test forms must be signed by a WFSC officer or the test chair. A "permission to test out of club" form also must be filled out and signed by a club officer or test chair. Most clubs require that the forms are received by them 2 weeks prior to the test date. Most applications for testing at area rinks are available at clevelandskating.com.
Permission Form
In order for a skater to participate in a USFSA sanctioned event at a club other than his/ her home club, this form must be completed by an officer of the skater's home club and sent to the club hosting the event.
